How To Get Hamad Hospital Health Card ?

Who May Apply For Health Card ?

Citizens and Residents of Qatar can apply for a Health card to access services at any Hamad Hospitals in Doha Qatar.

Health Card contains your ID number, your Nationality, photo and date issue and are available to all residents of Qatar.

To obtain Health card, visit the nearest Hamad Health Center to your residence.

Check with cash counter or Helpdesk. You need to fill out one form. you will be issued a PHCC health file number. Please take with you :

• Required To Get Health Card :

1. A Valid copy of Qatar ID.

2. A copy of your passport.

3. Proof of address : This is nothing but your utility bill.

4. One photograph of the blue plates outside of your building.
5. One photograph (4x3cm).
6. for infants less than one year born outside Qatar : In additon to the above, bring the vacciation card and birth certificate from your home country.
   and health card of mother and father.
7. For Household Employees(maids, nanies, driver) : Qatar Id copy, passprot copy, Photograph and copies of your sponsor health and Qatar ID.
8. You need to pay QAR 100 using debit card or credit card.
9. Note: They don't accept cash

How To Renew Your Health Card Onlne ?

Renewing Health Card can be very simple and easy to do.

You can renew your health by two ways :
1. Online by using hukoomi Portal.

2. By Visiting Hamad Health Center.

Renew Health Card Online :

• The first step is to go to the hukoomi Portal.
• The Second step, Start filling out the online form by entering all the information.
• Next Step, enter your Qatar ID number and select Renew (update Expiry Date) in the Inforamtion page.
• Next, Click on Number of required years to renew the card and enter the mobile number.
• You cand now enter your email id to get the receipt voucher.
• Or Enter your cell phone number if you select Yes to receive SMS notification.
• Click Next to go to the Payment Details then click on pay.
• The final step is to pay the appropriate fees and submit the application.

Fees :

fees depend on type of applicants.

• For Qatar Adult : QAR 50
• For GCC nationals, Wheather adult or children : QAR 50
• For Resident, Wheather audlt or children : QAR 100
• For Domestic Staff : QAR 50
